Brigadier Leonora Bamuza-Phetlhe is facing an internal investigation after issuing a top security clearance to General Major Pat Mokushane.
This also happened before the assessment panel made a recommendation in this regard.
Mokushane replaced General Major Makhele in June when he was removed from the post without explanation.
Sally de Beer, the SAP spokesperson, confirmed that an internal investigation is under way, but no further information is provided.
