More than 20 000 cases were reported where patients in state hospitals were disadvantaged due to staff negligence.

Chris Hani hospital is currently the most dangerous hospital with more than 4,200 cases in 30 months. Steve Biko hospital in Pretoria had almost 1 800 cases.

Gwen Ramakgopa, the responsible minister, acknowledged that most cases are due to negligence, inability of personnel, human failures and systems that collapse.

What is even more worrying is that training hospitals register the highest cases and this is where prospective doctors are trained. These cases often lead to civil claims against the hospitals and the state.
